Sammi Jo Rae Midcap Koch, 38, of Bolivar, Ohio, passed away on June 18, 2026.
Sammi was born on February 10, 1988, and their life remains cherished by family, friends, and others who knew them.
Family members noted that Sammi was preceded in death by parents Dale Midcap and Joni Leinhard Koch, stepfather Richard Koch, and grandfather John Verne Lienhard.
Sammi is survived by brother Scott Midcap and Jessica of Brewster, stepsiblings Ryan Koch, Marcia Koch, and Laura Koch, nieces and nephews Nathan, Bronson and Jalyn, Jermaine Jr., Stanley, and Rozlyn, grandmother Nancy Lienhard, and many aunts, uncles, and cousins.
A memorial service was listed for Monday, June 29, 2026, at 6:00 PM at Koch Funeral Home of Gnadenhutten, 244 N Walnut St, Gnadenhutten, OH 44629.
